  • A statement, made under penalty of perjury, that you have a good faith belief the material was removed due to a mistake or misidentification of the material.

  • A statement that you consent to the jurisdiction of the federal district court for the judicial district where your address is located. If your address is outside the United States, you consent to jurisdiction for any judicial district in which rwlynch.com may be found.
  • A statement that you will accept service of process from the person who provided the original DMCA notification or an agent of such person. We will then forward your counter-notification to the original claimant.
